Medical Insurance: Offered through Great West Health Care. GCLI pays 100% of the base plan premium toward the premium amount for the employee. This benefit is available to Full Time Employees Only as a Base Plan or Choice Plan

Aflac Insurance: Offered through AFLAC. This benefit is available to Full and Part Time Employees. This benefit includes supplemental insurance to a major medical insurance policy. This includes Accident, Cancer, Hospital Confinement, Short-Term Disability and Life insurance. The premium amounts will vary, based on individual qualifications and policy selection. An AFLAC representative will contact you prior to the conclusion of your 90-day introductory period.

Long Term Disability: (Pays 60% of your salary) Offered through Mutual of Omaha. This benefit is available to Full time Employees Only. This benefit becomes available after 90 days of disability. GCLI pays the entire premium amount for you.
Cafeteria Plan/Section 125: (Flexible Spending Account) Allows eligible employees to set aside a specific pretax dollar amount for unreimbursed medical, dental, vision, over the counter items, and dependent care expenses. Also allows employees premiums for medical, dental and vision to be subject on a pretax basis.

401K Plan: Offered through Paychex. This benefit is available to Full and Part Time Employees. There is no waiting period for new employees to participate.

To qualify and sustain eligibility, you must be at least 21 years of age and have worked a minimum of 1000 hours during previous plan year.
Bereavement Leave: (Allows for 16 hours of paid leave for a death of an immediate family member) This benefit is available to Full and Part Time employees immediately upon hire. Members of immediate family include spouses, parents, brothers, sisters, children, grandchildren, grandparents, and parents-in-law. This time off is not deducted from the employees Paid Time Off.
Paid Time Off: (Eligible after 6 months of full time employment) This benefit is available to Full Time Employees only. See direct supervisor for PTO category and total hours eligible to earn.
